Once your surgeon has advised the need for surgery and you have discussed any issues or concerns you may have, you will receive Hospital forms to fill out. Please inform the Hospital of any specific medical conditions/allergies or any special care needs you may have. Accommodation preference (single, two-bedded and four-bedded rooms with en suites available) may also be requested.
